Psychiatric rehabilitation is a field that involves training, coordinating, supervising and managing the actions and activities of others to achieve their goal becoming independently reintegrated back into society and their former routines before the events of their particular ailment through a strengths-based focus[1]. The specific focus is on the process of reintroducing personal and social functionality in those diagnosed with a mental illness, whether it be from after hospitalization, time in prison, etc.[2]. These clients usually experience mild to severe psychiatric dysfunction[3]. Psychiatric Rehabilitation Programs (PPRs) offer a combination of individual work on achieving personal goals, group facilitated rehabilitation activities, field trips, and more[4]. This field involves the teamwork of a variety of healthcare professionals such as psychiatrists, nurses and multiple different types of counsellors[5]. Typically, the occupation includes the use of psychotherapy and medication[6].

[edit] Job Duties and Responsibilities

Psychatric Rehabilitation Counsellors organize and plan treatments and goals for clients. It is the rehabilitation worker’s job to provide personal assistance to the client(s). This is done through a biopsychosocial framework. Help and guidance are by the rehabilitation worker. For example, the worker is responsible for the arrangement and referral of medical and mental health services, as well as occupational or educational programs. The worker assists clients in managing their daily life demands, and helps them develop skills to do so independently. To maintain the information needed to assess and help clients, it is the rehabilitation worker’s duty to receive as much useful information and relevant sources as possible. This information is collected from networking and collaborating with other resources in the community. This is work is done alongside other various job titles, so communicating with supervisors and peers is routine. Together, the rehabilitation worker and the other professional will assess the client and create a treatment regime to complete as a team. Assessment and treatment of clients is achieved through guiding, motivating, consulting and monitoring clients. When working with clients, the duties often include assisting in finding employment, skill development, and other ways to integrate the client into the community[7]. While interacting with clients and other professionals, it is a crucial duty to document the details of such[8].

Typically, in the beginning of the day, there would be a staff meeting ordered by the agency director. During this meeting, the sharing treatment plans of clients and progress reports may commence. On a typical work day, the counsellor would use the basic office tools necessary to perform day-to-day administrative tasks. This includes: documentation, scheduling and spreadsheet software, the internet, fax machines, the smartphone and the telephone[9]. Communication is also done face to face between clients and colleagues of a variety of professions. Depending on the schedule and needs of clients, there can be large amount of appointments to cover in a day. Tasks for the day can involve receiving updates from medical teams, meeting with potential employers for a client, arranging appointments for a client to meet with a specialist, and more[10]. Work is mostly done in an environmentally controlled condition. There would be variation in conditions of the job depending on the setting that the counsellor is employed in. Such settings include schools, independent-living facilities, prisons, private practice and more[11]. Sometimes, the intervention occurs in the client’s home environment[12]. Typical PRP programs run 5 days a week from about 9am-2pm[13]. The clients in the program would participate in daily classes, offsite trips, and 1-on-1 rehabilitation with psychiatric rehabilitation counsellors[14]. It is recorded by 43% of rehabilitation workers report that the job typically requires 40 hours a week. Both physical and mental energy is spent on the repetition of certain activities, which include adding information to the computer based treatment log checking and documenting client activity [15].

[edit] Educational Requirements and other qualifications

In order to enter the rehabilitation profession, advanced credentials are necessary to provide the therapy that is needed (Learn.org). The minimum requirement is a Bachelor’s degree and/or diploma program in psychology or social work (Education Portal), but a Master’s degree in counselling is most likely required to get a positon as a rehabilitation worker. Besides achieving a Master’s degree, to become a therapist, special licensing is needed. The national competency-based association that provides the credential to become a Registered Professional Counsellor (RPC) is the Canadian Professional Counsellors Association (CPCA)[16]. To become a member of the CPCA, two years of experience shadowing an approved Clinical Supervisor is needed. The purpose of this licensing is to protect the public by ensuring that mental health counsellors are competent in providing help and support[17]. In the province of Quebec, counselor licensure is granted by the Ordre des conseillers et conseilleres d’orientation et psychoeducateurs et psychoeducatrices du Quebec (OCCOPPQ)[18]. Due to the wide-ranging duties and responsibility for this position, between three and five years of experience is required. Minimal on-the-job training is provided due to the expectations of skill gained through years of experience[19].

Active listening is a very important skill. This is done by both listening and asking questions, and being considerate by not interrupting the person speaking. Active listening is also expressed through body language. Attention must be placed on both interpersonal and intrapersonal skills to assure that the counsellor presents him/herself in a manner that promotes openness and respect. Self-control is another relevant skill in the occupation due to difficult situations that may arise[20]. When difficult situations arise, it is important to maintain composure. Empathy is a required personal characteristic to gain rapport with the clients and further motivate them complete their goals. This trait is the fuel behind promoting the strengths based focused that would prove successful in assisting the client’s growth of autonomy.

[edit] Relevance of Psychology Undergraduate degree

Courses on Health Psychology – learning about the effects of psyche on immunity will help with the overall rehabilitation needed for the client, since this will help with all aspects of life. The study of complementary and alternative medicine will prove beneficial in teaching clients skills that will help with their health on their own time. These skills encourage autonomy. Complementary and alternative health therapies are used across multiple cultures, so this would broaden your awareness and ability to help the diverse clients that make up Ontario.

Courses on Abnormal Psychology – it is important to be knowledgeable of the assessment and treatment of behavioural disorders. This course gives you an understanding of the diagnostic criteria and the etiological factors of various psychopathologies. It also teaches about individual differences, which will be witnessed within the diverse environment that will be the rehabilitation agency. This provides insight on therapy and counselling, which is a crucial part of the psychiatric rehabilitation position.

Social Psychology – Learning about group behaviour is especially important in a job where you work alongside a team. Learning about the influence that society has on and individual will teach you about the structure of dynamics and the importance of culture. This is all necessary to create rapport with clients and maintain a functional relationship with coworkers.

According to the national salary data on payscale.com, a rehabilitation counselor earns between $34,947 and $67,981 per year. The average salary of a rehabilitation counselor is $47,023 per year[21]. Since a psychiatric rehabilitation specialist has the option to work in both public institutions and private practices[22], variance in salary may be due to the type of agency of the employer. For example, in the United States, the counselors who are paid the highest salary work at local or state government agencies; this followed by hospitals and residential care facilities[23]. Counselors employed in a group practice or private practice have the highest counselling salaries than other industries that employ counselling services[24].

In the Canadian Occupational Projection System, the projection of demand of counsellors are grouped with psychologists, social workers, clergy, and probation officers. This is due to some occupations being too small to be analyzed individually[25]. Over the 2013-2022 period, there will be an excess demand, with job openings expected to total 60,895[26].
